ultrarapide. Cependant, pour les tissus anisotropes et visco\u00e9lastiques tels les muscles, l&rsquo;\u00e9lastographie SWE souffre de deux inconv\u00e9nients qui limitent son application. 1) les mesures SWE 2D ne sont pas capables de prendre en compte l&rsquo;anisotropie (AN) musculaire due aux fibres qui se d\u00e9placent dans l&rsquo;espace 3D, ce qui induit un biais de mesure. 2) Bien que la viscosit\u00e9 est une propri\u00e9t\u00e9 cruciale pour le muscle, les technologies SWE actuelles ne permettent pas de mesurer la viscosit\u00e9. Par cons\u00e9quent, l&rsquo;objectif du projet INNOVAN est de s&rsquo;attaquer \u00e0 ces deux inconv\u00e9nients, afin d&rsquo;\u00e9tendre fortement le potentiel de SWE. L&rsquo;ambition de ce projet multidisciplinaire est de proposer des innovations en imagerie US afin 1) d&rsquo;obtenir une meilleure compr\u00e9hension fondamentale du comportement musculaire et 2) d&rsquo;effectuer des applications cliniques importantes. \u00c0 cette fin, nous avons r\u00e9uni un consortium de grands groupes de recherche en physique, biom\u00e9canique, physiologie et href=\"https:\/\/anr.fr\/Projet-ANR-23-CE19-0040\" target=\"_blank\" rel=\"noopener\">FATUS<\/a><\/h4><p>Le projet FATUS a pour premier objectif de lever le verrou de la quantification des infiltra graisseux intramusculaires par ultrasons gr\u00e2ce \u00e0 une m\u00e9thode innovante de beamforming. Une \u00e9tape de validation par IRM viendra confirmer l\u2019atteinte de cet objectif par un d\u00e9veloppement des s\u00e9quences sp\u00e9cifiques pour la quantification graisseuse. Par la connaissance et la cartographie de ces infiltras, le troisi\u00e8me objectif ambitionne de mieux comprendre le comportement m\u00e9canique des muscles en pr\u00e9sence de graisse par l\u2019am\u00e9lioration des techniques d\u2019\u00e9lastographie ultrasonore. Le quatri\u00e8me objectif s\u2019oriente autour d\u2019une caract\u00e9risation et d\u2019une quantification volumique des muscles. Le dernier objectif vise des applications cliniques pour l\u2019aide au diagnostic de la sarcop\u00e9nie li\u00e9e au vieillissement. Ce projet pluridisciplinaire propose donc de partir d\u2019innovations dans le domaine de l\u2019imagerie m\u00e9dicale ultrasonore, pour aller vers une meilleure compr\u00e9hension fondamentale du comportement 3D du muscle, puis vers des applications cliniques particuli\u00e8rement importante chez les personnes \u00e2g\u00e9es De nombreuses autres applications cliniques seront envisag\u00e9es \u00e0 l\u2019issue du projet. Afin de le mener \u00e0 bien, nous avons r\u00e9uni une \u00e9quipe pluridisciplinaire de chercheurs leaders dans leurs domaines (physique, industriel, biom\u00e9canique, QUANTUM<\/h4><p>L\u2019objectif de cette \u00e9tude est de d\u00e9velopper un protocole exp\u00e9rimental d\u2019ERM et d\u2019\u00e9lastographie-US qui quantifie les propri\u00e9t\u00e9s m\u00e9caniques de tumeurs greff\u00e9es en sous-cutan\u00e9 chez le petit animal et soumises \u00e0 une compression uniaxiale. Nous souhaitons ainsi avoir une meilleure connaissance des caract\u00e9ristiques m\u00e9caniques tumorales et \u00e9valuer leur lien avec les composantes de pression (pression solide, pression fluide interstitielle), mais aussi leur relation avec les mesures histologiques (quantit\u00e9 de collag\u00e8ne, architecture).<\/p>\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<section lumi\u00e8re \u00e0 l&rsquo;int\u00e9rieur des tissus biologiques, l&rsquo;imagerie m\u00e9dicale optique non invasive profonde est tr\u00e8s difficile. L&rsquo;imagerie acousto-optique est une technique couplant ultrasons et lumi\u00e8re qui permet de r\u00e9cup\u00e9rer le contraste optique \u00e0 des profondeurs de quelques centim\u00e8tres avec une r\u00e9solution millim\u00e9trique. Les progr\u00e8s r\u00e9cents de l&rsquo;imagerie acousto-optique utilisent des impulsions ultrasonores focalis\u00e9es courtes, souvent moyenn\u00e9es sur plusieurs centaines ou milliers d&rsquo;impulsions. Comme la fr\u00e9quence de pulsation des sondes commerciales est limit\u00e9e \u00e0 environ quelques cycles d&rsquo;ultrasons toutes les 100 ms, l&rsquo;acquisition d&rsquo;une image acousto-optique prend g\u00e9n\u00e9ralement plusieurs dizaines de secondes en raison du nombre \u00e9lev\u00e9 d&rsquo;impulsions acoustiques d&rsquo;excitation. La ventilation m\u00e9canique assure des \u00e9changes gazeux ad\u00e9quats mais peut produire des effets n\u00e9fastes qui sont associ\u00e9s \u00e0 une augmentation de la mortalit\u00e9, de la morbidit\u00e9 et des co\u00fbts de sant\u00e9. Le taux d&rsquo;occupation des lits de soins intensifs en France est \u00e9lev\u00e9 (&gt;80%) et toute strat\u00e9gie visant \u00e0 pr\u00e9venir un s\u00e9jour prolong\u00e9 est cruciale et plus que jamais d&rsquo;actualit\u00e9 dans le contexte de la pand\u00e9mie de COVID-19. \u00c9viter un effort insuffisant ou excessif du diaphragme est devenu un objectif essentiel dans la prise en charge des patients sous ventilation m\u00e9canique. Cette strat\u00e9gie, appel\u00e9e ventilation protectrice du diaphragme, ne peut \u00eatre mise en oeuvre sans outils fiables et facilement accessibles pour \u00e9valuer le diaphragme, et plus largement, les muscles inspiratoires et expiratoires accessoires. La surveillance des muscles respiratoires au sein de l\u2019USI est complexe et les m\u00e9thodes de r\u00e9f\u00e9rence reposant sur la mesure de la pression transdiaphragmatique sont invasives, peu disponibles et n\u00e9cessitent une forte expertise. Afin de surmonter ces limitations, un nouveau concept d&rsquo;imagerie ultrarapide, ultraportable et d\u00e9mat\u00e9rialis\u00e9e a \u00e9t\u00e9 r\u00e9cemment d\u00e9velopp\u00e9. Cette approche induit un changement de paradigme en d\u00e9passant les limites inh\u00e9rentes \u00e0 la miniaturisation du mat\u00e9riel et ouvre de nouveaux horizons pour l\u2019analyse d\u2019image, faisant notamment appel \u00e0 l\u2019intelligence artificielle. Il s&rsquo;agit d&rsquo;une occasion unique d\u2019impl\u00e9menter des biomarqueurs ultrasonores de qualit\u00e9 sup\u00e9rieure dans un facteur de forme ultraportable. En s\u2019appuyant sur cette plateforme et sur les avanc\u00e9es r\u00e9centes dans le domaine des ultrasons ultrarapides, ce projet vise \u00e0 d\u00e9velopper une solution ultraportable sp\u00e9cialement con\u00e7ue pour l&rsquo;\u00e9valuation de la structure et de la fonction des muscles respiratoires. Nous utiliserons ce nouvel outil pour am\u00e9liorer les connaissances sur la structure et la fonction des muscles respiratoires, avec l\u2019objectif final d\u2019optimiser la prise en charge de ces les Jeux olympiques, courir vite est la t\u00e2che motrice la plus r\u00e9pandue, le 100 m sprint \u00e9tant l&rsquo;un des \u00e9v\u00e9nements les plus attendus. Pourtant, la r\u00e9alisation de tels sprints de course n\u00e9cessite \u00e0 la fois des capacit\u00e9s athl\u00e9tiques extr\u00eames et un syst\u00e8me musculosquelettique robuste afin de limiter le risque de blessure. Les blessures musculaires des membres inf\u00e9rieurs sont en effet la principale cause d&rsquo;interruption d&rsquo;entra\u00eenement ou de comp\u00e9tition sur la sc\u00e8ne internationale. Dans ce contexte, la France pr\u00e9sente la particularit\u00e9 d&rsquo;\u00eatre historiquement forte dans les sports ax\u00e9s sur la vitesse et reconnue pour la qualit\u00e9 des travaux de recherche effectu\u00e9s pour la compr\u00e9hension des performances de sprint. Ce programme de recherche est men\u00e9 en \u00e9troite collaboration avec les f\u00e9d\u00e9rations fran\u00e7aises d&rsquo;athl\u00e9tisme, de rugby et de sports de glace (bobsleigh). FULGUR rassemble les plus grands experts mondiaux en biom\u00e9canique musculaire, recherche sur la force et le conditionnement, l&rsquo;imagerie clinique, les comportements de sant\u00e9 et l&rsquo;apprentissage automatique appliqu\u00e9s \u00e0 la tr\u00e8s haute performance sportive afin de poursuivre un triple objectif:<br \/>\u2022 Evaluer la m\u00e9canique du sprint au niveau du centre de masse et des segments d&rsquo;articulation en vue de quantifier la charge de travail des structures sp\u00e9cifiques \u00e0 ces \u00e9chelles dans des conditions r\u00e9elles; dans cette recherche de nouveaux param\u00e8tres d&rsquo;\u00e9lastographie en utilisant l&rsquo;imagerie ultrasonore ultrarapide sont mis au point pour pr\u00e9venir les blessures musculaires.<br \/>\u2022 port\u00e9<\/b>: projet DIAMAND<\/h4><p>La mesure de param\u00e8tres physiologiques est essentielle pour le diagnostic d\u2019une pathologie chronique ou le suivi de son \u00e9volution en r\u00e9ponse \u00e0 un traitement. L\u2019\u00e9valuation de ces param\u00e8tres dans la vie quotidienne est un d\u00e9fi technologique pour la m\u00e9decine personnalis\u00e9e. Depuis quelques ann\u00e9es, les syst\u00e8mes port\u00e9s se multiplient, que ce soit \u00e0 des vis\u00e9es m\u00e9dicales ou pour le bien \u00eatre. Ils sont performants en terme d\u2019ergonomie et d\u2019autonomie, mais la robustesse d\u00e9cevante de la mesure reste probl\u00e9matique d\u2019un point de vue m\u00e9dical. L\u2019approche port\u00e9e par le projet DIAMAND est d\u2019associer \u00e0 ces diff\u00e9rentes modalit\u00e9s une mesure par ultrasons de param\u00e8tres physiologiques, et notamment du diam\u00e8tre de l\u2019art\u00e8re. L\u2019objectif est de proposer et de tester une architecture et une strat\u00e9gie d\u2019acquisition minimalistes pour obtenir des mesures de diam\u00e8tre d\u2019art\u00e8re par ultrasons avec une grande pr\u00e9cision (qqs \u03bcm). Cet aspect \u00ab minimaliste \u00bb permettra de limiter l\u2019encombrement global du dispositif, sa consommation \u00e9nerg\u00e9tique, et la quantit\u00e9 de donn\u00e9es g\u00e9n\u00e9r\u00e9es, pour pouvoir par la suite int\u00e9grer cette mesure dans un dispositif multimodal de mesure de param\u00e8tres physiologiques.<\/p>\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<section Cette recherche combinera deux techniques \u00e9chographiques r\u00e9centes d\u00e9velopp\u00e9es par nos soins ces derni\u00e8res ann\u00e9es: l&rsquo;\u00e9lastographie et le Doppler ultrasensible, MUSCLOR<\/h4><p>Les d\u00e9veloppements r\u00e9cents dans le domaine de l&rsquo;\u00e9lastographie visent \u00e0 quantifier de nouvelles propri\u00e9t\u00e9s m\u00e9caniques des tissus, compl\u00e9mentaires du module de cisaillement repr\u00e9sentant les propri\u00e9t\u00e9s \u00e9lastiques lin\u00e9aires d&rsquo;un milieu quasi incompressible. Dans ce contexte, la mesure de la non-lin\u00e9arit\u00e9 \u00e9lastique des tissus a \u00e9t\u00e9 r\u00e9cemment propos\u00e9e sur la base de l&rsquo;acousto\u00e9lasticit\u00e9. Jusqu&rsquo;\u00e0 pr\u00e9sent, la plupart des d\u00e9veloppements th\u00e9oriques acousto\u00e9lastiques ont suppos\u00e9 que les tissus \u00e9taient isotropes. Cependant, cette hypoth\u00e8se forte ne tient pas dans tous les tissus humains, tels que les muscles qui sont g\u00e9n\u00e9ralement consid\u00e9r\u00e9s comme transversalement isotrope (TI). Dans ce projet, la th\u00e9orie de l&rsquo;acousto\u00e9lasticit\u00e9 en milieu TI quasi incompressible est d\u00e9velopp\u00e9e et mise en \u0153uvre exp\u00e9rimentalement sur fant\u00f4mes TI et sur des tissus musculaires <em>ex vivo<\/em> et <em>in vivo<\/em>. En pratique les \u00e9volutions de la vitesse des ondes de cisaillement est \u00e9valu\u00e9e par rapport \u00e0 la contrainte statique uniaxiale exerc\u00e9e sur le milieu donnant acc\u00e8s\u00a0 au param\u00e8tre \u00e9lastique non lin\u00e9aire A (qqs centaines de kPa). Ces travaux ouvrent la voie \u00e0 une caract\u00e9risation plus approfondie des propri\u00e9t\u00e9s m\u00e9caniques musculaires ainsi qu&rsquo;au d\u00e9veloppement d&rsquo;un nouveau biomarqueur BPALP<\/h4><p>L&rsquo;\u00e9lastographie est une technique d&rsquo;imagerie r\u00e9cente (en IRM ou en \u00e9chographie (US)) qui permet de quantifier les propri\u00e9t\u00e9s biom\u00e9caniques (BMP) des tissus mous. L&rsquo;\u00e9lastographie c\u00e9r\u00e9brale a \u00e9t\u00e9 largement \u00e9tudi\u00e9e r\u00e9cemment et a permis, principalement <em>ex vivo<\/em>, de mieux comprendre les pathologies tumorales ou d\u00e9g\u00e9n\u00e9ratives. Mais cette technique souffre d&rsquo;inconv\u00e9nients pour la pratique clinique quotidienne. En ultrasons, en raison de l&rsquo;os du cr\u00e2ne, il n&rsquo;est pas possible d&rsquo;\u00e9tudier de mani\u00e8re non invasive les BMP c\u00e9r\u00e9brale sans neurochirurgie. En IRM, il est n\u00e9cessaire de faire vibrer la t\u00eate \u00e0 partir d&rsquo;un actionneur externe, ce qui rend le diagnostic clinique quotidien difficile. Ainsi, les BMP du cerveau sont mal connues <em>in vivo<\/em> et sont mal utilis\u00e9es comme biomarqueur pour le diagnostic du cancer ou la surveillance du traitement par rapport \u00e0 d&rsquo;autres organes. Nous proposons de d\u00e9velopper une m\u00e9thode d&rsquo;IRM originale sans vibrateur externe pour le diagnostic des tumeurs du cancer du cerveau avec une technique innovante issue de l&rsquo;imagerie ultrasonore, l&rsquo;\u00e9lastographie passive. Nous d\u00e9veloppons des s\u00e9quences IRM: sur les aimants cliniques, 3T pour la compr\u00e9hension des BMP et la pratique clinique quotidienne; et pr\u00e9cliniques, 7T pour la compr\u00e9hension fondamentale et le suivi du traitement de chimioth\u00e9rapie. Ceci est valid\u00e9 en regard par \u00e9lastographie passive ultrasonore, consid\u00e9r\u00e9e comme gold standard, en pr\u00e9clinique avec craniotomie et chez l&rsquo;homme pendant la
